Karachentsov becomes one of the leading actors of Lenkom, Zakharov attracts him to participate in many performances. In 1991 he was elected Secretary of the Union of Theater Workers of Russia. He is an academician of the Russian Academy of Cinematic Arts "NIKA". In 1994, together with his friend, stuntman and teacher Nikolai Astapov, he created the Nikolai Karachentsov School of Arts (ShIK) in Krasnoarmeysk, in which children studied in various directions - from dancing to painting, from fencing to horse riding. Karachentsov suffered a concussion and was taken to hospital. On March 3, 2017, he was discharged from the hospital.

In September 2017, the actor was diagnosed with an inoperable cancer tumor in his left lung. In early October 2018, he was hospitalized at the 62nd Oncology Hospital in Moscow.
